Dr. Steven Fish, MD
Dr. Steven Fish is a board-certified eye surgeon with over 8,000 surgeries performed across California. Known for his steady hand and calm demeanor, he has built his career on precision, trust, and cutting-edge technology—especially in laser-based procedures.
Dr. Fish's journey into aesthetic medicine began naturally through his experience with intense pulsed light (IPL) treatments in ophthalmology, primarily for dry eye and inflammation management. Intrigued by IPL's broader capabilities, particularly in addressing sun damage, redness, pigmentation, and skin texture, he expanded his professional focus to aesthetics, blending surgical precision with cosmetic care. Today, he offers physician-led IPL and OptiLift® treatments designed to restore clear, healthy, and naturally refreshed skin.
At Coastal Aesthetic Physicians, Dr. Fish combines surgical expertise with a patient-first approach, ensuring every treatment is safe, effective, and rooted in clinical precision. He lives in Encinitas with his wife, Madison, and their two young children. Outside the clinic, you’ll often find him chasing his kids at the park or squeezing in a quick round of golf before sunset.

Dr. Rachael Allen is a board-certified ophthalmologist whose precision with delicate facial anatomy naturally led her to the world of aesthetic medicine. During her ophthalmology residency, she discovered a deep passion for oculoplastic procedures, facial aesthetics, and skincare—sparking a journey that blends medical expertise with artistic finesse.
After proudly serving 11 years as an active-duty physician in the United States Navy, Dr. Allen transitioned her focus to medical aesthetics, bringing the same dedication and attention to detail to her aesthetic practice. She offers advanced treatments including Botox® and Xeomin® injections, microneedling, chemical peels, intense pulse light (IPL), and Lumenis OptiLift® therapies—all designed to help patients achieve refreshed, radiant, and natural-looking results.
Now based in Carlsbad with her husband, Dr. Thomas Allen, and their three energetic children, Dr. Allen balances family life with her mission to help others feel confident in their skin. Her signature approach? “A little laser, a little Botox, and a whole lot of glow.” ✨

As both a physician and a mother of two, Dr. Fish understands how meaningful it is to make time for yourself — and how small changes in skin health can make a big difference in daily confidence. She now focuses her aesthetic practice on IPL and OptiLift treatments, providing safe, effective, and physician-led options for skin rejuvenation.

Dr. Thomas Allen, MD
Dr. Thomas Allen is a board-certified emergency medicine physician with more than a decade of experience caring for patients in high-pressure, high-stakes environments. His years in the ER gave him a deep appreciation for preventive care and inspired his passion for aesthetic medicine, where he helps patients take charge of their confidence and well-being before problems arise.
At Coastal Aesthetic Physicians, Dr. Allen focuses on men’s aesthetic services designed for busy professionals and dads. He offers physician-led treatments including Botox® (“Brotox”) — and yes, he’s a fan himself — along with a straightforward, approachable perspective on men’s care. His goal is to normalize aesthetics for men, providing safe, effective options that enhance appearance while preserving a natural look.
Dr. Allen lives in Carlsbad with his wife, Dr. Rachael Allen, and their three energetic children. Outside the clinic and ER, he enjoys golfing, hiking, working out, and chasing his kids around the beach. His philosophy is simple: looking good and feeling good go hand in hand.

Every consultation is performed by a board-certified physician, who also provides your first treatment.
If any follow-up care is performed by an advanced-practice nurse practitioner or licensed clinician, a board-certified physician will always be available by phone and can evaluate you in person within 24 hours, if medically necessary.
We believe aesthetic care deserves the same level of oversight as any medical treatment.
